Sample requirements
Documentation Required for Non-Standard Samples
Non-standard samples require additional review and documentation. Depending on the sample type and circumstances, KINORA GENETICS (DNA) may require:
- a completed Non-Standard Sample Declaration
- valid photo identification
- birth or death documentation where applicable
- information describing the origin of the sample
- information describing how the sample was collected, stored and handled
- other laboratory-requested documentation
DNA recovery cannot be guaranteed
A non-standard sample cannot be guaranteed to produce a usable DNA profile.
If required information or documentation cannot be provided, KINORA may require the submitting party to acknowledge responsibility for the sample and the increased risk of testing failure.
Additional testing or re-sampling fees may apply if the sample cannot be successfully tested.
Non-Standard Sample Declaration
The declaration records the information the laboratory needs in order to assess whether a non-standard sample can be processed:
- Sample owner name
- Relationship to applicant
- Description of sample
- Source of sample
- How the sample was obtained
- Approximate age of sample
- Storage conditions
- Whether the sample may have been handled by other people
Before you mail anything
Please do not send a non-standard sample until KINORA GENETICS confirms acceptance and provides handling instructions. Submit the sample assessment first so the required declaration and documents can be reviewed.
