Bring to the appointment
- Government-issued photo identification for every adult donor
- Identification or birth documentation for any minor
- The reference or file number from the requesting authority, if you have one
- Any written instruction letter you received
For minors
A parent or legal guardian must be present and provide consent. Bring documentation showing the guardianship relationship where possible.
For overseas relatives
Collection abroad must be arranged in advance. Allow extra time for appointment scheduling, courier transit and customs handling. Confirm the relative's full legal name exactly as it appears on their identification.
